Reservations are required and may be made by phoning the church office at (330)602-6008 or by completing the reservation form below. PLEASE NOTE - TICKETS ARE ONLY AVAILABLE FOR THURSDAY NIGHT.
Please complete the entire form including full names of those who will be receiving tickets. You will need to list yourself if you are in need of a ticket. If you are requesting multiple tickets, please list each person's name seperated by a comma. You may list more than one name on a form.
For ticket requests for both nights, please use a seperate form for each night.
Children's meals may be requested for young children by listing those children in the Kids' Meal section. If the children would like the Adult Meal, list their names in the adult section.
